What caused the ParkTree Community Health Center data breach?

Parktree has added a website notice to their homepage that provides more details about the incident.
According to these sources, PCHC experienced a network outage on February 7, 2024. In response, PCHC launched an investigation to learn more about the incident and whether any confidential patient information may have been affected. Through this investigation, PCHC learned that on February 7, 2024, an unauthorized third party gained access to files containing confidential patient information.
After discovering that sensitive consumer data was accessible to unauthorized third parties, ParkTree Community Health Centers reviewed the breached files to determine what information was exposed and which consumers were affected. PCHC completed this process on July 9, 2024. Compromised information varies by individual but may include name, date of birth, health insurance identification number, medical record number, contact information, gender, medical information, etc.
On August 21, 2024, ParkTree Community Health Center sent data breach notification letters to all individuals affected by the recent data security incident. These notification letters included a list of what victims’ information was compromised.
